North East Essex Day Services
Pathway2 "… a better quality of life" service offers support to people aged 17-65 who are experiencing mental ill health and are registered with a GP living in North East Essex. The service aims to support recovery through reducing social isolation and supporting people to engage with their communities through a variety of methods.
Our Service offers:
- Person centred planning
- Community Bridge Building
- Befriending and peer support
- Self-esteem and assertiveness groups
- Support to access the Towards Inclusion Grant Scheme
- Drop-in services; and
- Support for self-help groups.
Recovery is...
Recovery is an individual journey. You take steps towards your goals, take responsibility, and have hope. It is about looking at the whole person and not just the illness, making choices and gaining a sense of wellbeing. The role of our Service is to facilitate, encourage, support and provide information and practical support that enables you to make informed choices and access the right kind of support and mainstream opportunities.
Our Community Bridge Builders
Your Community Bridge Builder will talk to you about the different ways we can meet your needs by providing practical time limited support. Our goal is to support you to aim for a better quality of life. We will support you to identify your aspirations, make a plan how to achieve them using mainstream community facilities, and then we will support you to achieve them.
Our Service will...
Support you to access mainstream activities within the community. For example, if you want to, our Service can help you to:
- take a course or support you if you are already taking one
- exercise and joining sports clubs for fun and fitness
- find support from faith and cultural groups
- find support to sort out your housing
- meet new people and build new friendships and
- go to museums, galleries, concerts, movies, or the theatre or
- volunteer in your community
- join a neighbourhood or community group or
- build your self-confidence

What is a Person Centred Plan
Your Bridge Builder will make an appointment to see you and help you make your Person Centred Plan. This plan is an agreement between your Bridge Builder and you about what your goals are and how the service can help you achieve them. There may be a short wait for some types of support that we offer but your Bridge Builder can let you know the availability and process to receive that support.
How Can I Access The Service?
You can self refer into the service by telephoning 01206 572504.
Agencies and professionals can also refer clients into the service by completing a referral form and faxing it to 01206 564825.
